High Blood Pressure

Must be <140/90​ with or without treatment for one year certification

>140/90 but less than 179/109 qualifies for 3 month certification

Cardiac Events (heart attack, stent/angioplasty, open heart surgery, heart failure, etc.)

A stress test and/or echocardiogram are required every 1-2 years​

Bring a statement or most recent office visit note from your cardiologist

Diabetes

A1C within 3 months must be <10%, bring labs

Bring a statement or most recent visit note from your treating provider that documents adequate blood sugar control without hypoglycemia

Insulin dependence: bring this form completed by your treating provider within 45 days

Obstructive Sleep Apnea (OSA)
-
Must attest to no daytime sleepiness, episodes of apnea, or falling asleep at the wheel, and show evidence of treatment compliance​

Documentation of annual assessment from sleep doctor

Compliance report from your CPAP machine, within the last month must show 70% compliance >4 hours

Vision
-
Visual acuity must be 20/40 in each eye and both together (with or without glasses/contacts)​

Hearing
-
No more than 40 dB (5500-2000 Hz) hearing loss in your better ear, with or without hearing aids​

Mental Health (depression, anxiety, ADD/ADHD, etc.)

Bring your most recent office visit note from the treating provider. ​

The note should state that your condition, treatment, and medications are safe with commercial driving duties
